We read Google’s map results for “med spa san antonio” on 31 August 2026, with the search location set to San Antonio. Three clinics held the pack, and the one with more than twice the reviews of the leader sat in third place.

The number that does not do what people assume

554 reviews finished behind 256.

That is more than double the volume, at a rating four tenths higher, one seat lower. It is the third Central Texas pack we have read this month where the most-reviewed business is not the first result, and it is worth being precise about what that does and does not mean.

It does not mean reviews are unimportant. Every clinic here has hundreds of them, which tells you the entry price for this pack rather than the finishing order. What it means is that once several competitors are all well-reviewed, review count stops being the thing that separates them — and whatever is separating them is not printed on the card.

Proximity to whoever is searching is the obvious candidate, and it is the one nobody can optimize away. (What actually sits behind map pack order.)

Both of the top two are on the same freeway

Seats one and two are both I-10 addresses, several miles apart, and both carry the same 3+ years chip. Seat three is on Broadway, a different part of the city entirely.

For a metro the size of San Antonio, a three-result pack is a very small window onto a very large market, and where the searcher stands moves it. A clinic in Stone Oak and a clinic in Alamo Heights are not really competing for one list — they are competing for different lists that happen to share a query. What a clinic should take from this

Do not read a competitor’s review count as your target. In this pack it would have pointed you at the wrong business.

Read the cards instead for what is missing on yours: the category, whether the years chip is there, whether the address and hours are right, whether the services are itemized. Those are the parts you control, and in a pack where everyone has hundreds of reviews they are what is left. (The profile fields most clinics leave half-filled.)
